Using the Forms

These forms are editable PDF documents with buttons at the top of the form to print and to submit by e-mail to our parish office.

You can directly enter your information into the PDF documents on-line.

Once you have filled in the details, click the "print form" button to print a copy for your own records. Then click the "submit by e-mail" button and the form will be e-mailed to the parish office.

If you do not have a mail application on your computer, you will be prompted to save the information into a file (*.xml).

You can then attach this file to an e-mail in your Internet browser (Internet Explorer, Firefox, Safari, etc).

Alternatively, you can mail the printed form to our parish office.

Confirmation

Children's Confirmation

Confirmation is the next sacrament of initiation after Children's baptism. In the diocese of Broken Bay, this sacrament is generally celebrated when the child is in year 2 at school. The celebration is normally held in third term in St Leonard's Church, Naremburn and in St Thomas' Church, Willoughby.

An information pack is prepared in May / June each year. It includes a letter outlining the process and key dates, an enrolment form for the sacrament, and a parishoner information form for family details and contact information.

The pack is distributed to all Year 2 students at St Philip Neri School Northbridge and St Thomas' School Willoughby and those attending catechist classes at Castle Cove, Northbridge and Willoughby Public Schools. Information packs are also available from June onward at the parish office, at the rear of the churches and by download from this website (see documents dowlnloads in the right-hand column).

Adult Confirmation

In the diocese of Broken Bay, adult confirmation is celebrated by the Bishop at the Waitara Cathedral on Pentecost Sunday each year. Please contact the parish pastoral associate, Paul Mason, for further details, on 9438 1011.
