Using the Form

This form is an editable PDF document with buttons at the top of the form to print and to submit by e-mail to our parish office.

You can directly enter your information into the PDF document on-line.

Once you have filled in the details, click the "print form" button to print a copy for your own records. Then click the "submit by e-mail" button and the form will be e-mailed to the parish office.

If you do not have a mail application on your computer, you will be prompted to save the information into a file (*.xml).

You can then attach this file to an e-mail in your Internet browser (Internet Explorer, Firefox, Safari, etc).

Alternatively, you can mail the printed form to our parish office.

Confirmation

Children's Confirmation

Confirmation is the next sacrament of initiation after Children's baptism. In the diocese of Broken Bay, this sacrament is generally celebrated when the child is in year 2 at school. The celebration is normally held in June in St Philip Neri and St Leonard's. An information pack is distributed in March each year to Year 2 students at St Philip Neri and those attending catechist classes at Northbridge Primary. Information packs are also available from the parish office, at the rear of the churches and by download from the Access Forms page of this website.

Adult Confirmation

In the diocese of Broken Bay, adult confirmation is celebrated by the Bishop at the Waitara Cathedral on Prentecost Sunday each year. Please contact the parish pastoral associate for further details, on 9438 1011.
