Using the Form

This form is an editable PDF document with buttons at the top of the form to print and to submit by e-mail to our parish office.

You can directly enter your information into the PDF document on-line.

Once you have filled in the details, click the "print form" button to print a copy for your own records. Then click the "submit by e-mail" button and the form will be e-mailed to the parish office.

If you do not have a mail application on your computer, you will be prompted to save the information into a file (*.xml).

You can then attach this file to an e-mail in your Internet browser (Internet Explorer, Firefox, Safari, etc).

Alternatively, you can mail the printed form to our parish office.

Enrolment form and key dates have been distributed by e-mail to those who made their First Reconciliation. These are also available in the foyer of our Churches and by download from this website.
